Registration for the 2022 CSTM Annual Conference is now Closed

Please note all registrants will be required to provide proof of COVID 19 vaccination to attend the conference.

Cancellation Policy
 Notice of cancellation must be made in writing to the Conference Secretariat at conference@transfusion.ca . A $60.00 (plus 13% HST) cancellation fee will apply for conference registrations cancelled until May 20, 2022. After this date, no refunds will be issued. 

Registration Policies

 Registration fees must be paid in full before you will be admitted to the conference. All registration fees are subject to 13% Harmonized Sales Tax (HST). All fees are listed and must be paid in Canadian dollars (CAD)

1. Registrations will be accepted until May 20, 2022.   There will be no on-site registration.  Substitutions may be made until May 27, 2022 by writing to the Conference Secretariat at conference@transfusion.ca.
2. Registration fees must be paid by VISA or MasterCard.
3. A confirmation notice detailing your registration will be sent to you immediately upon completion of this registration form. If you do not receive this confirmation within 24 hours, please contact us at conference@transfusion.ca.
4. Recording may occur through the conference; by agreeing to these policies, conference attendees consent to filming, photography and sound recording as delegates, speakers or exhibitors at the conference.
5. All registration rates are subject to availability.
6. Completion of this form constitutes a binding agreement to the policies listed above.

Special Events

One complimentary ticket to each event is included with your registration up to capacity limits.  Register early to guarantee your special events tickets. Please indicate your attendance by selecting the attendee ticket for the events you will be attending. If you would like to bring a guest, please purchase a guest ticket for the event(s).

 Member Registration Codes

Regular members are required to enter the membership discount code in the "discount code" box at the bottom right of the form in order to receive the membership rate listed above. The membership code was provided to all active members on January 08, 2022.  For those who obtained their membership after January 08, 2022 the code will be sent with your membership confirmation.  

 If you need to renew or sign up for new membership, please visit the CSTM website here; alternatively you can bundle your membership with member rate full conference registration by selecting the appropriate Registration plus CSTM Membership ticket.
 
 Students

To receive the student rate, proof of full-time enrolment must be provided. A valid student card or a copy of current course schedule will be accepted. This can be emailed to conference@transfusion.ca .  Upon receipt of proof of enrollment, a discount code will be provided to reduce the regular rates to student rates.  Do not register until you have received the discount codes.
